Texas Chapter 121 disclosure
Required by Texas Business & Commerce Code Chapter 121. The full version, addressed to you by name, is emailed automatically the moment you start a placement. This is the same disclosure in general form, before you have given us any contact information.
1. Services provided
Placement OS (operated by AI Perfect Fit Senior Living Placement, Houston, TX) provides a personalized referral matching service that connects families with licensed assisted living, memory care, and independent living communities in Texas. Our service includes intake assessment, community matching, application coordination, and follow-up through move-in.
2. Who pays the fee
Our referral and placement service is paid solely by the placing community upon a successful move-in. Your family pays nothing for our service, no service fee, now or ever. Separately, a community itself may require a refundable reservation deposit to hold a bed; that deposit is paid to the community under its own policy and is not a fee for our service.
3. Your right to stop, without penalty
You may stop using our referral service at any time, for any reason, without cost, penalty, or further obligation, and pursue placement independently. If you tell us to stop, we will let every community we already referred you to know that you have stopped using us.
4. Our list is not every community
The list of communities we give you may not include all of the communities in your area that meet what you told us you need and prefer. There are communities we have not reviewed, communities not in our network, and communities that may be a good fit for your family even though they never appear in our results. You are always free to look beyond our list.
5. How communities are ordered in your results
Some communities in our network have signed an agreement to pay us a placement fee if your family moves in. Those communities appear first in the list we show you. We tell you this plainly so you can weigh it. Every community we show you is matched to what you told us you need, we never hide a community because it does not pay us, and you are free to choose any community, in our list or not, at no cost to you.
6. No conflicts of interest
We do not refer families to communities in which we hold ownership or financial interest beyond the referral fee disclosed above. We do not accept compensation from any party other than the placing community.
7. No power of attorney
We do not hold, request, or accept power of attorney over any consumer, and we do not take possession of consumer property, financial accounts, or legal authority.
8. Licensed or permitted communities only
We check every community before we put it in front of your family, and we keep a record of that check, against the Texas Health and Human Services Commission's own published licensing directory. Some smaller homes hold a boarding home permit from their city or county instead of an HHSC license; where that is the case we check the local permit list and tell you plainly which kind of home you are looking at.
Privacy
We collect what you tell us during intake (care needs, location, budget, timeline, and your contact information) and what a community tells us when you apply. We use it for one purpose: matching your family to licensed communities and coordinating your application.
We do not sell your information, to anyone, ever. Your contact information is shared only with the communities you choose to apply to from your shortlist. We do not resell your phone number to call centers or third-party marketers.
You can ask us to delete your information at any time by emailing admin@seniorliving.expert. We will remove it from active matching; records already sent to a community you applied to are that community's own record and are deleted on your request to them directly.
Terms of service
Placement OS is a free referral and matching service for families searching for senior living in Texas. We are not a medical provider and nothing on this site is medical advice; care decisions are between your family and the community and its licensed staff.
Community information (licensing, capacity, certifications) is sourced from Texas Health and Human Services and CMS public records and refreshed periodically; always confirm current availability and pricing directly with a community before deciding.
By using this site you agree we may contact you about your placement request. You may opt out at any time, per the Chapter 121 disclosure above. Texas law governs these terms.
